Forest Lake Case Management, (formerly known as Behavioral Health Home (BHH) services), is a service offered by Nystrom & Associates that helps those with a mental health diagnosis meet their health and wellness goals. We provide you with a team of workers to assist with linkage to resources, identifying barriers to accessing and receiving services, care coordination, transitional care, and individual and family support. Our case management services identify mental health, chemical health, physical health, and wellness needs through a holistic, person-centered approach. Our case managers will meet with you in person and/or through phone calls at least once per month.

What Can I Expect From Forest Lake's Case Management Services?

A team consisting of a Registered Nurse (RN), Systems Navigator (Case Manager), and a Qualified Health Home Specialist (Peer Support Specialist) will meet with you in your home, in the community, or by phone to assist you with identifying and achieving your wellness goals. Case managers will provide a contact at least once a month to assist you with referrals, resources, and care coordination. You may work with this team until your individual needs are met.

Your Forest Lake Case Management team works in coordination with other services you might have like ARMHS, psychiatry, therapy, DBT, ADT, nutrition, primary care services, and more. Case management services can be provided in conjunction with all other mental health, physical health, and substance use services that you have, with the exception of case management.

About Forest Lake, MN

Forest Lake is a city in Washington County, Minnesota, United States, located 27 miles northeast of Saint Paul. The 2019 population is 20,933. Forest Lake began as a stop on the St. Paul and Duluth Railroad. The first train reached Forest Lake on December 23, 1868. The lake was so named for the abundant timber that lines its shores. Forest Lake Township was organized on March 11, 1874; the first one-room school was built that year at the former location of city hall (220 N. Lake Street). The city of Forest Lake was incorporated on July 11, 1893 with 175 residents. In 2001, the city annexed the surrounding former Forest Lake Township.
