White Bear Lake Case Management
White Bear Lake Case Management, (formerly known as Behavioral Health Home (BHH) services), is a service offered by Nystrom & Associates that helps those with a mental health diagnosis meet their health and wellness goals. We provide you with a team of workers to assist with linkage to resources, identifying barriers to accessing and receiving services, care coordination, transitional care, and individual and family support. Our case management services identify mental health, chemical health, physical health, and wellness needs through a holistic, person-centered approach. Our case managers will meet with you in person and/or through phone calls at least once per month.
What Can I Expect From White Bear Lake's Case Management Services?
A team consisting of a Registered Nurse (RN), Systems Navigator (Case Manager), and a Qualified Health Home Specialist (Peer Support Specialist) will meet with you in your home, in the community, or by phone to assist you with identifying and achieving your wellness goals. Case managers will provide a contact at least once a month to assist you with referrals, resources, and care coordination. You may work with this team until your individual needs are met.
Your White Bear Lake Case Management team works in coordination with other services you might have like ARMHS, psychiatry, therapy, DBT, ADT, nutrition, primary care services, and more. Case management services can be provided in conjunction with all other mental health, physical health, and substance use services that you have, with the exception of case management.

About White Bear Lake, MN
White Bear Lake is a city in Ramsey County in the state of Minnesota, United States. A small portion of the city also extends into Washington County. The population was 23,769 at the 2010 census. The city is named after its largest lake, White Bear Lake. American writers have delivered differing versions of the legend that explains the origin of the name. In her book Indian Legends of Minnesota, Mrs. Carl T. Thayer writes that "It is said that a Sioux maiden fell in love with a Chippewa brave. She, the daughter of the Chief, on learning that her father planned war against the Chippewa, ran to her lover and warned him. The brave went alone into the Sioux village to ask for peace and the hand of the maiden. Before the Chief would agree, the Chippewa would have to do a brave deed."
