A Message From Nystrom & Associates regarding Coronavirus (COVID-19)


Appointment Information

  • If your provider is able to meet with you over video or phone from your home, staff will contact you the day prior to inform you of the details. You will also be sent an email the evening before your appointment with additional instructions.
  • Appointments can be done by phone or video. If the patient does not have video capability, then the appointment will take place by phone.
  • IF BY PHONE: Your provider will call you at your appointment time.
  • IF BY VIDEO: Your provider will explain the details of this while on the phone with you during your appointment.
  • All first-time appointments for psychiatry can take place over video from your home. For this option, you must have access to a device with video capability — such as a smartphone, tablet, or computer with a webcam.
    • You will be sent an email the evening prior to your appointment with details on how to connect
  • If you do not have access to a device with video capability, you must come to the clinic in-person for your psychiatry intake appointment.
    • However, if you are sick or experiencing any signs of illness, please do not come to your appointment. Please call (1-844-NYSTROM) or email (contactus@nystromcounseling.com) to reschedule. Any charges for late cancellation, for this reason, will be waived.
  • All brand new patients must complete the Fee & Policy Agreement prior to their first appointment.
  • Please complete the applicable intake paperwork prior to your appointment:

  • Depending on the service you are scheduled for, there may be intake paperwork that you need to complete prior to your appointment. See below under the “Intake Paperwork” bullet point for details if you are scheduled for any of the following services:
    • Psychological Testing
    • Adult Day Treatment
    • Substance Use Disorder Orientation
    • Nutrition

Insurance Information

Nystrom & Associates is working closely with each insurance company to ensure that telephonic and tele-video services are covered by each insurance plan. Most insurance plans (UBH/Medica, Blue Cross Blue Shield, Health Partners, Preferred One, etc) have already announced they are issuing temporary coverage for telephonic and tele-video appointments during the COVID-19 pandemic. We encourage patients to keep their appointments and know that Nystrom & Associates is advocating and will continue to advocate for each insurance company for coverage for all tele-appointments that take place during the pandemic.  

  1. Call the Human Resources department of your past or current employer and ask if you have active coverage. Ask how long you’re covered for. Usually, coverage ends on the last day of the month that you were employed.
  2. If your coverage is ending, ask if you will be able to enroll in continuation coverage (COBRA). Your employer is generally legally obligated to offer you COBRA continuation coverage if your coverage ends and this coverage starts the day immediately after your coverage ends.
  3. If you don’t have the option for COBRA or you’re interested in other options, consider applying for coverage with MNsure. In response to the COVID-19 pandemic, MNsure has opened a special enrollment period to obtain health insurance on the private marketplace. Starting on March 23rd, any Minnesotan can apply for health insurance on MNsure for coverage starting on April 1, with a deadline of April 21st to get coverage under this special enrollment period. Here are the broad eligibility criteria:
  • This special enrollment period is for eligible Minnesotans who do not have current health insurance.
  • You do not need to be sick to qualify.
  • If you are currently enrolled in a plan through MNsure, you cannot use this special enrollment period to change plans.

Copay Information

Safe Passage Letter

There is currently no requirement to carry a Safe Passage letter to/from your appointment with us. However, if you’d like to have a copy of it, you can find it here: Safe Passage Letter.
