Plymouth Case Management
Plymouth Case Management, (formerly known as Behavioral Health Home (BHH) services), is a service offered by Nystrom & Associates that helps individuals with a mental health diagnosis meet their health and wellness goals. We will provide participants with a team of workers to assist with linkage to resources, identifying barriers to accessing and receiving services, care coordination, transitional care, and individual and family support. Plymouth Case Management identifies mental health, chemical health, physical health, and wellness needs through a holistic, person-centered approach. Our case managers will meet with you in person and/or through phone calls at least one time per month.
What Can I Expect From Plymouth Case Management?
A team consisting of a Registered Nurse (RN), Systems Navigator (Case Manager), and a Qualified Health Home Specialist (Peer Support Specialist) will meet with you in your home, in the community, or by phone to assist you with identifying and achieving your wellness goals. Case managers will provide a contact at least once a month to assist you with referrals, resources, and care coordination. Participants may work with this team until their individual needs are met.
Your Plymouth Case Management team works in coordination with other services you might have like ARMHS, psychiatry, therapy, DBT, ADT, nutrition, primary care services, and more. Case Management can be provided in conjunction with all other mental health, physical health, and substance use services that you have, with the exception of case management.

About Plymouth, MN
Plymouth is a suburban city in the U.S. state of Minnesota 15 miles (24 km) west of downtown Minneapolis in Hennepin County. It is located on the western edge of the Minneapolis suburbs. According to the 2019 Metropolitan Council estimate, Plymouth's population is 79,768. Once named for Medicine Lake, the city's name was chosen by Hennepin County Commissioners during the county's inception. Plymouth's history can be traced back to the pre-Columbian period around 1400 to 1500 AD. The original inhabitants were the Dakota. Their encampment was at the north end of Medicine Lake. The name Medicine Lake is derived from the Dakota word Mdewakanton, meaning "Lake of the Spirit." The Dakota named the lake after a warrior overturned his canoe and his body was never recovered.
