Saint Anthony Case Management
Saint Anthony Case Management, (formerly known as Behavioral Health Home (BHH) services), is a service offered by Nystrom & Associates that helps those with a mental health diagnosis meet their health and wellness goals. We provide you with a team of workers to assist with linkage to resources, identifying barriers to accessing and receiving services, care coordination, transitional care, and individual and family support. Our case management services identify mental health, chemical health, physical health, and wellness needs through a holistic, person-centered approach. Our case managers will meet with you in person and/or through phone calls at least once per month.
What Can I Expect From Saint Anthony's Case Management Services?
A team consisting of a Registered Nurse (RN), Systems Navigator (Case Manager), and a Qualified Health Home Specialist (Peer Support Specialist) will meet with you in your home, in the community, or by phone to assist you with identifying and achieving your wellness goals. Case managers will provide a contact at least once a month to assist you with referrals, resources, and care coordination. You may work with this team until your individual needs are met.
Your Saint Anthony Case Management team works in coordination with other services you might have like ARMHS, psychiatry, therapy, DBT, ADT, nutrition, primary care services, and more. Case management services can be provided in conjunction with all other mental health, physical health, and substance use services that you have, with the exception of case management.

About Saint Anthony, MN
St. Anthony, also known as Saint Anthony Village, is a city in Hennepin and Ramsey counties in the U.S. state of Minnesota. At the 2010 census the population was 8,226 of whom 5,156 lived in the larger Hennepin County part of the city and 3,070 in the Ramsey County part. St. Anthony's origins date to 1838, when Franklin Steele, a storekeeper at Fort Snelling, made a claim on the land east of St. Anthony Falls. Steele did not begin developing the land until 1848, but it quickly became a center of milling and trade much like the neighboring town of Minneapolis on the land west of the falls. In 1858 the town was formally organized as the Township of St. Anthony.
